Gender-affirmative treatment of minors is driven primarily by ideological convictions and financial interests, while the scientific evidence base is uncertain and a return to evidence-based medicine is necessary.
The entire field of gender-affirmative treatment of children and adolescents must urgently return from ideology to evidence-based discourse.
The text analyses a new US government report that raises allegations of billing fraud against clinics and WPATH that treat minors with puberty blockers. It argues that financial interests and the creation of chronic patients in paediatrics are central drivers of gender-affirmative care. The author refers to political measures by the Trump administration that cut federal funding for such treatments, as well as lawsuits by the FTC against WPATH for consumer fraud. In addition, the scientific evidence base of the treatments is described as uncertain, with reference to international studies in Scandinavia and the United Kingdom. The text calls for the field of gender-affirmative treatment to be freed from ideology and brought back to evidence-based discourse. It warns that ideological convictions and monetary interests may have displaced medical expertise. Finally, Germany is urged to critically observe developments in the USA and to question its own guideline practice.

The Trump administration is increasing pressure on clinics that continue to prescribe puberty blockers and cross-sex hormones to minors. A new US report accuses doctors, clinics, and WPATH not only of questionable medical standards but also of financial interests and potential billing fraud.

In the United States, the Trump administration is continuing its restrictive course against gender-affirming treatments for minors. This means that children and adolescents who suffer from their biological sex characteristics should no longer be treated with puberty blockers and cross-sex hormones. Sex-reassignment treatments in the sense of a transition should henceforth be reserved for adults only.
Billing fraud with puberty blockers
After the US Department of Health and Human Services commissioned a report in 2025 to determine how irresponsible the practice of gender-affirming treatments is in the country, another report was released this month. The current document, titled “Wolves in white coats,” deals with allegations of billing fraud against doctors and clinics that continue to prescribe puberty blockers and cross-sex hormones to minors at the expense of state-funded health systems, despite state decrees and legal bans in individual US states.
For billing, diagnostic codes such as “Endocrine disorder, not otherwise specified” were used, instead of those for gender identity disorders (ICD-10) or gender incongruence and gender dysphoria (ICD-11). The World Professional Association for Transgender Health (WPATH), the international organization for all professionals working in the field of sex reassignment, reportedly recommended this in professional presentations—corresponding PowerPoint slides are documented in the report.
From the new report
As Politico reports, the present report was released two days after the termination of Medicaid funding for hospitals that provide sex-reassignment care for minors. Early in his term, US President Donald Trump issued decrees ending state support for such treatments for minors. This includes the requirement that state healthcare providers such as Medicaid should no longer cover the costs of puberty blockers, cross-sex hormones, and sex-reassignment surgeries.
More profit in pediatrics
The new report attempts to show that greed for profit was likely one of the key drivers for many providers to enter the field of gender-affirming treatments. Pediatrics is normally not considered an area in a clinic’s portfolio where significant profits can be made, as there are hardly any patients with a need for chronic treatment. The authors calculate:
“This dynamic of the ‘bound patient’ stands in stark contrast to the patterns of usual pediatric care, which—as data from the National Center for Health Statistics show—focuses predominantly on acute or self-limiting illnesses. Examples include a single doctor’s visit to treat an ear infection or a one-time hospital stay due to an acute rotavirus infection.
A study on outpatient care, which analyzed data from the years 2008 to 2013, found that 82% of pediatric patients did not have chronic illnesses; patients with multiple chronic conditions did have more frequent doctor contacts, but they represented a minority of the patients treated in this care setting.
In contrast to typical single appointments for acute complaints or preventive examinations, a child or adolescent with gender dysphoria enters a process that includes regular endocrinological consultations, laboratory tests (hormone levels, monitoring for side effects such as bone density, liver function, lipid profiles, as well as fertility counseling), psychological follow-up care, and possibly ultimately irreversible surgical procedures.
Or also expressed in dollar amounts:
"Estimates of the costs illustrate the scale of these revenues. According to a comprehensive analysis from 2022, based on billing data from privately insured transgender patients over a period of nearly three decades, cross-sex hormone therapy generates average annual costs for payers of $545 (androgens), $735 (estrogens), and $16,385 (for the more expensive GnRH analogs used for puberty suppression).
The lifetime total costs for a patient who begins treatment as a minor can amount to $25,000 to $75,000, even without surgical procedures. For comparison: average healthcare expenditures for children under 18 typically amount to approximately $3,000.
These figures reflect the amounts covered by health insurance and do not take into account patient cost-sharing, which can further increase total costs. Although these expenditures may appear moderate on an annual basis, they occur annually throughout the patient's entire adult life, thereby generating cumulative revenues for the healthcare system.
Surgical procedures significantly increase the financial potential. An average cost of $12,680 is incurred for a 'top surgery' (mastectomy with masculinization of the chest area in women), covered by health insurance, plus cost-sharing amounts of $2,244. Similarly, a mammoplasty causes costs of $17,426 per patient for the insurer, plus additional cost-sharing of $1,223. 'Bottom surgery'—vaginoplasty in men and phalloplasty in women—is more complex and is often performed in multiple stages.
The average total costs per person amount to $53,645 for a vaginoplasty and $133,911 for a phalloplasty. These are the average amounts covered by health insurance (payer perspective) without considering cost-sharing, which averaged approximately $2,624 for vaginoplasty and $3,982 for phalloplasty.
Furthermore, follow-up procedures may be required for these surgeries due to complications (such as urethral strictures, fistulas, loss of sensation, or cosmetic corrections), each of which entails additional time in the operating room, hospital stays, and follow-up treatments."
Gender-affirmative care is in accordance with human rights
All of this lagged far behind the fact that the gender-affirmative model, as presented by transgender activists, their allies in queer activism, and in politics (especially within the Democratic Party) and associations such as WPATH (who consistently identify with the Democratic Party), was portrayed as the only one that is in accordance with human rights. Careful psychotherapeutic exploration and the consideration of alternatives to a medical transition pathway were increasingly marginalized, being stigmatized as "gatekeeping" and "conversion therapy."
Nevertheless, the number of skeptical parents and medical professionals in the USA and worldwide grew, questioning whether these developments are correct. After systematic investigations into the evidence for gender-affirmative treatments involving puberty blockers began in countries such as Finland, Sweden, and the United Kingdom from 2019 onwards, it became apparent on how uncertain and scant a foundation this treatment approach rests. Since then, voices in medicine and psychology demanding a revision of this model have been increasing.
FTC accuses WPATH of consumer fraud
The new report is not the only measure pushed by the Trump administration. As early as June of this year, it became known that the Federal Trade Commission (FTC), together with several US states, is filing a lawsuit against the World Professional Association for Transgender Health (WPATH). The accusation: WPATH’s gender-affirmative Standards of Care, which are considered the authoritative reference worldwide in the field of sex reassignment, maliciously deceived minors willing to transition and their parents with false promises about the safety and effectiveness of treatments with puberty blockers and the like. As a US federal agency, the FTC is responsible for consumer protection as well as protecting the US market from monopolies and unfair competition.
Almost simultaneously with the release of the Department of Health report, WPATH’s defense against the FTC’s lawsuit was announced. In it, the defendant rejects the accusations, as its treatment recommendations are not directed at end consumers, but at medical professionals. The latter, in turn, bear individual responsibility for their decisions. At the same time, WPATH admitted that its Standards of Care represent a scientifically grounded opinion of proponents of gender-affirmative interventions, but not the final, conclusively established state of the art.
Rather, the scientific debate is not yet concluded, and positions advocating a more cautious approach to puberty blockers are equally legitimate. This is likely to surprise all those who, in recent years, have had to face accusations that they deny the existence of transgender people or spread hate, are unscientific, unethical, and so on.
Department of Justice to initiate investigations
Following the publication of the report “Wolves in white coats,” Vice President J.D. Vance is now urging the Department of Justice to initiate investigations for billing fraud against the clinics in question. Ahead of the midterms in November, this is certainly a welcome moment for the Trump administration to demonstrate that they are doing what they were elected for. After all, the unresolved conflicts surrounding the transgender issue, which also includes ideological ignorance of the consequences of irreversible experimental interventions on minors, was an important reason why voters in 2024 voted for the Republicans under Donald Trump.
In Germany, one should look closely at what is happening in the USA. The German S2k guideline “Gender incongruence and gender dysphoria in childhood and adolescence – Diagnosis and treatment” largely corresponds to WPATH’s Standards of Care. The organization that is now pointing out in the USA that these do not represent the concluded state of discussion in medicine. One should also carefully consider whether one encourages billing and diagnostic creativity at professional events, which is well documented.
The entire field of gender-affirmative treatment of children and adolescents must urgently return from ideology to evidence-based discourse. The decisive question is: Does the fervor of the trans-activist scene and its medical allies have less to do with medical expertise and more to do with monetary interests – their own and those of the pharmaceutical industry?

World Professional Association for Transgender Health (WPATH) erhebt.
Justizministerium soll Ermittlungen einleiten
In Deutschland sollte man genau hinzuschauen, was nun in den USA passiert.
