Loud protest and the use of tribal language on both sides prevent a substantive, evidence-based debate about medical treatments for children and adolescents with gender incongruence.

The debate should therefore really take place exactly where things get complicated: with evidence, individual circumstances, risks, benefits, and capacity to consent.

The text describes a protest and counter-demonstration in Hamburg on World Children's Day, at which an event by the association Frauenheldinnen on the topic of trans medicine was met with loud counter-protest. The author concedes that the positions of the Frauenheldinnen can provoke legitimate dissent, and points out that the event title already contains a conclusion. At the same time, he criticises the counter-protest, which attempted to drown out the event with air horns, chants, and the slogan "TERFs into the Elbe." He describes this language as harmful to democratic culture and demands that minority rights must also withstand the freedom of expression of political opponents. The author refers to the statement by Green Party member of parliament Miriam Block, who described the demands of the Frauenheldinnen as deadly, and calls for differentiation here as well. He emphasises that the real debate should take place around evidence, risks, benefits, and capacity to consent, rather than around tribal language. The text overall advocates for a substantive engagement with medical treatments for children and adolescents.

on site "TERFs into the Elbe": When the Protest Itself Damages the Debate In Hamburg, the dispute over trans medicine escalated on World Children's Day when a critical protest event met loud counter-protest. Between air horns, chants, and mutual accusations, the actual question fell out of sight: How can one argue factually about medical treatments of children and adolescents? The conflict shows how difficult it has become to withstand political opposition without wanting to silence the other side. Sebastian Schnelle 30.09.2026 2 Counter-demonstrators shielded the assembly of the "Frauenheldinnen" from the street with large fabric banners. Photo: © Frauenheldinnen e.V. On September 20, two sides that can barely speak to each other anymore met at a demonstration in Hamburg. The association Frauenheldinnen held a protest and information event at the Hamburger Jungfernstieg under the title "Trans medicine makes you sick − for a lifetime." The occasion was World Children's Day. On the other side, counter-protest formed. The Frauenheldinnen belong to that feminist current that defines sex by physical characteristics and critically views medical measures in children and adolescents in particular. Their positions range from resistance to the Self-Determination Act to fundamental criticism of gender-affirming treatments, especially in minors. That these positions provoke opposition is now known and not surprising. Equally unsurprising is that an event whose title already contains the conclusion "Trans medicine makes you sick − for a lifetime" does not have to be understood as a neutral medical information event. However, it does not follow from this that every form of counter-protest would be acceptable. The Frauenheldinnen themselves explicitly announced the Hamburg event as a protest action. Their starting point is the conviction that children and adolescents must be protected from medical interventions that they consider too early, too irreversible, or not sufficiently scientifically substantiated. Among the speakers in Hamburg was Chris Brönimann, who was previously known under the name Nadja Brönimann and, according to his own account, lives again as a man after a detransition. The Frauenheldinnen announced Brönimann as the central speaker of the event. Brönimann's story is undoubtedly personally profound. On X, Brönimann describes his own experience in a post as follows: "It's not about rejection, but about responsibility. From my own experience I know the magnitude and know: A hasty trans medicine in adolescence often does not heal, but carries the risk of making you sick for a lifetime." This certainly represents a legitimate position, but it alone cannot answer how the treatment of other people with gender incongruence should be assessed. According to the Frauenheldinnen, up to 200 people took part in the counter-protest. The association reports that counter-demonstrators had left their own assembly area, drowned out the Frauenheldinnen event with air horns and chants, and among other things chanted "TERFs 1 into the Elbe." In addition, a participant had been spat on. The association published an open letter to the Hamburg Parliament on this matter. Severe damage to the democratic culture The phrase "TERFs into the Elbe" is more than a mere pointed political slogan. Even if such a slogan in the context of a demonstration is not automatically to be understood as a concrete call to violence against a specific person, it uses a language that employs violence and intimidation as imagery. This is a path that does serious damage to the democratic culture in this country. Those who accuse the Frauenheldinnen of excluding trans people should not simultaneously try to make their event inaudible through intimidation. Those who claim to be demonstrating for the rights of a minority do not have to approve of the freedom of assembly and expression of their political opponents, but they should tolerate it. Whistles are not arguments. "Shut up" is not an argument. And "TERFs into the Elbe" is not a political answer to a medical or feminist point of contention. Especially revealing is the fact that the dispute was apparently not only conducted between activist groups. According to the Frauenheldinnen, the Green Party member of parliament Miriam Block also spoke at the counter-demonstration. She reportedly stated: "Stop your incitement! You are not protecting anyone. Your demands endanger human lives, they kill." Differentiation would be appropriate here as well. The claim that certain political demands "kill" is an extremely severe political attribution. However, the same principle applies to the Frauenheldinnen themselves: those who claim that "trans medicine" makes children and young people ill for life must also differentiate and substantiate their statements with reliable data. The dispute does not become more scientific by one side morally loading its opponents to the maximum negative extent. Besides all the activist noise, there are indeed legitimate questions: How safe are certain medical interventions? How are young people accompanied diagnostically? What role do psychological stresses and other illnesses play? How reversible are individual measures? What consequences do puberty blockers, hormones, or operations have? And how are people counseled who later change their decision? The dispute should therefore actually take place exactly where it becomes complicated: with evidence, individual prerequisites, risks, benefits, and capacity to consent. Instead, camp terms are increasingly dominant. Some speak of "trans ideology," others of "TERFs." Some speak of "trans medicine" as if it were a uniform block, others treat any criticism of it as an attack on trans people. Both make a factual debate difficult. The Hamburg events are therefore exemplary of a larger context.
