Integrative oncology does not replace evidence-based medicine but complements it with psychosocial and complementary measures to strengthen patients' self-efficacy and protect them from the dangers of pseudomedicine.
Patient empowerment or self-efficacy, that is the core point, and it does not hinge on an acupuncture needle or a globule, but it hinges on the realization. I can do something here and I can become active.
The text defines the term integrative oncology as the complementing of evidence-based cancer therapy with scientifically grounded complementary methods such as nutrition, exercise, and psychology. It clearly distinguishes this from alternative medicine, which is considered ineffective or harmful and often relies on placebo effects or a lack of attention. A central theme is the societal loss of health literacy, which leads patients to gullibly resort to ineffective remedies. The speaker emphasizes the duty of medical professionals to communicate honestly in order to maintain trust and prevent conspiracy theories. The core of healing is identified as the promotion of self-efficacy and the active participation of patients in their recovery. The text warns against the uncertainty in society and the dependence on external healing promises, which weaken one's own body awareness.

Hello and a warm welcome to a new episode of the Cortices Podcast. My name is Jürgen Hübner. Today we are speaking with an expert who, like no other, brings clarity to the jungle of complementary medicine offered and supposedly gentle healing methods. Dr. Jutta Hübner, Professor of Integrative Oncology at the University of Jena, specialist physician in Internal Medicine, Hematology, and Oncology. As chair of the Working Group on Prevention and Integrative Oncology of the German Cancer Society, she has been working for years to distinguish between effective support and dangerous charlatanism. In conversation with Bettina Eder, the discussion centers on the term integrative oncology, a field often misunderstood, situated between evidence-based medicine and esoteric promises. Professor Hübner makes clear where scientifically grounded complementary medicine can sensibly supplement cancer therapy, for example through nutrition, exercise, and psychological support, and where alternative methods can cause harm, particularly through loss of time and dangerous interactions. She speaks about the challenges in doctor-patient communication, the societal loss of health literacy, and the problematic role of social media as a source of knowledge. It also becomes clear that placebo effects, care, and self-efficacy have their place, but not at the expense of honesty and scientific rigor. As always, I wish you much fun and great pleasure in gaining insight. Professor Hübner, a warm welcome. I am truly very happy that I may interview you today and that you have taken the time to speak with us. And I would propose that we first define exactly what we are talking about today. Because I can imagine that the term integrative oncology can easily be misunderstood when one thinks of the terms evidence-based medicine, naturopathy, alternative medicine, and complementary medicine. Yes, it is indeed the case that the term is not even uniformly defined. It is usually said, well, it is just evidence-based or scientific medicine plus alternative medicine. And these are two such contradictory things that you simply cannot put into one package. So I would like to define it differently. Let us try to start with alternative medicine. It is often defined as a substitute for, but I believe that most people use alternative medical methods in parallel with the actual therapy. At least in the case of more serious illnesses. They are thus running on two tracks, without the two tracks being connected to each other like in a proper railway. Alternative... alternative to complementary medicine. Correct, that is a huge contrast. Alternative medicine consists of things that are either clearly refuted or at least unproven scientifically, often also scientifically inconceivable to work, and which partially have a significant potential for harm. Complementary medicine consists of accompanying methods that are, however, evidence-based. They often come from naturopathy but not only. So if you take, for example, my body, yoga, or something like that, it is not a classic form of naturopathy. And that then fits with the evidence-based actual therapy. I am an oncologist, so cancer therapy, and it supplements it in a sensible way. So it is always again away from the field of alternative and nonsensical so-called quackery. Whatever terms one may choose. It is important to me that we understand under integrative medicine the sensible things that help the patient. That is very good conventional medicine on our side. Much side-effect management, communication, is decisive for good integrative medicine. We do nutrition, we have sports, we have psychology. And then a nice cherry on top of well-done complementary medicine can be added
to come. It probably also depends on the fact that many patients, especially in oncology, as I can imagine, due to the severe side effects, still wish for a gentle alternative or a gentle supplement. Yes, so many components come together there. What are, so to speak, the factors that lead patients to go on a search? That is often quite understandable. First of all, the desire. Highly complex therapies are carried out with many side effects, so that one first goes on a search: is there not something gentle that quickly makes me healthy, that has a 95 percent probability of healing or something like that, is completely legitimate. Most people then already think that doctors, when they recommend such strenuous things as chemo, radiation, and the like, already have a good reason, but still look on this other track, and health literacy is a problem in Germany. We have a very low one compared to other countries, and are not able to really evaluate it properly. That is why it is perhaps also so important that we doctors and other professional groups, pharmacists, nursing staff, communicate clearly and give patients a clear navigational environment with which they can then cope. How do you then communicate that to patients and women patients? If you have, I would say, a demand with little evidence, I cannot now add globules XY so that my nausea is not so bad. How do you deal with that, and do you also have tips on how medical professionals in general could communicate better? Yes, first of all, I believe that we all in the health professions have a very high obligation to honesty and integrity. Cheating, lying, and the like is absolutely out of the question, because nothing is worse than when patients no longer trust us. The hope that we accompany them and that we do it seriously is quite essential in serious illnesses, in cough, cold, hoarseness, okay. But even there one must be honest, because if patients start to distrust all health professionals, we find ourselves in a very dangerous situation. Then conspiracy theories and other things suddenly take hold. That means, if I notice during the patient’s anamnesis that there are a few things that are not evidence-based, that may even be truly dangerous, then I address them and try to educate. No patient wants to harm themselves. And I do not know any patient with a serious illness who wants a non-helpful therapy. Because they spend money on it, but above all they invest time. And that is a significant loss in a serious illness if they unnecessarily use their time and often, and that is the exciting thing about complementary medicine, we have for what the patient wants to achieve a good naturopathic nutritional or sports answer. And that simply has to be placed in that position first. Then there are indeed patients who say, yes, but I am now taking the globules from Schorn, I got them from my very dear family doctor. And I feel better. Now the question is, what does one do in that situation? And there I also educate that one can call it a placebo effect. I believe that something else is actually behind it. That is an effect of care. The family doctor finally listened, what the oncologist perhaps no longer did. And she took care. That is care and that does good, that helps. And that can be explained very well to the patient. And with that one can first strengthen the patient-family doctor relationship. And I think it is good that this bond works. Then one can say, I do not believe that it is really due to the globules. If it helps you and the problem is solved, then you can
but I am happy to continue. If at some point they come to the idea that perhaps it is not due to the pellets, but because I have the feeling that I am doing myself a good thing when I take them in the morning, then they can also try something else than the globules. And then they might discover that it is quite magical when you allow yourself to do something good for yourself. And then they can swim a little free from it. They do not have to, but they can. And I simply want to open this door. Then one can so to speak say, you make the patient in that case more or less their own detective, so that they themselves also deal with themselves more mindfully. You understand it correctly. Perhaps the core secret of complementary medicine. The actual disease healing of side effect management. We do that well enough with all our chemical stuff. But this mental component, I can actively contribute something here. And I also start to understand what my inner voice says or what I sometimes do not want and what it is afraid of. This technical term does not exist in German. Patient empowerment or self-efficacy, that is the core point and it does not bind to the acupuncture needle or to a globule, but it binds to the realization. I can participate here and I can become active. I believe that is an extremely important realization that can really help a lot and is a super tip that I believe one can give to all healers. That is what is fascinating to me, why I so much like being an oncologist. Many ask, how can one bear that, always these dying patients. It is the great thing that they really want to become active. I believe that other, chronically ill patients also fundamentally want to do that. And to promote and support that a little is wonderful. Exactly, so I have also made the experience. I do not know if I am right with that, if it is only a personal feeling or if it is actually true that people have less and less contact with themselves, with their body and also less and less trust in their own self-healing powers. If I notice now with small trivialities like a bruise, I usually know, it will go away on its own. The body can do that completely on its own. But I have the feeling that more and more people then think they must also, coming back to the globules, somehow take globules so that it disappears faster or so that it goes away well at all. Do you also see that people want external help and are less and less self-effective themselves than they perhaps were 10, 20, 30 years ago? I find that a very difficult question. But that is certainly also because I do not have a broad general practice or something like that, but a very specific consultation point. So people who come to me are rather those who are actively on the go, otherwise they would not end up with me at all. I believe that this tendency exists in our society right now, in a very high and further increasing uncertainty in all areas. And that also affects the topic of health. And precisely here I see the problem, that for example many pediatricians already start with globules or other things. Acupuncture is also such a story, because it suggests to people. Actually it is the one you described, it gets better on its own. But then it is attributed to, because I took something. So the body obviously constantly needs support in the sense of self-optimization, faster healing and similar, all the advertising for any over-the-counter medications and dietary supplements aims at that. And people start to believe that more and more. And because they no longer try anything else, they also no longer gather the experience that it
it can also function on its own. Precisely for that reason, I believe that doctors and other healthcare professionals must be much clearer in their language. It is no coincidence that the German appears to be the person with the most doctor contacts in the entire world. Yes, but this also presents an opportunity for the healthcare system again. To initiate another about-face. I believe we urgently need to work on the topic of health literacy, and this must become a subject in schools. But for that, we would first have to train teachers accordingly or have other ideas. I believe we cannot send the doctor to the schools anymore. We have too few of them, and not enough nursing staff either. But we need an idea there. It is not acceptable that we teach all possible subjects, but not exactly the most important one. Yes, this brings us back to the core topic. We need critical thinking always and everywhere. That would be the exciting part, perhaps a school education, not just organized by subjects, just as I think one can also conduct the university studies differently, but rather, so to speak, outlining problem areas and showing how biology and physics and chemistry, and also in school one could teach a bit about medications, i.e., pharmacy or pharmacology, to people. So, so to speak, attempting thematic designs to teach the various sciences and thereby also to promote critical thinking. I find this a very exciting approach. A really good idea. I believe that this could excite young people, if I look at what is done in project weeks, what is done in semester or seminar papers in the 12th grade, what comes out of youth research, then I believe that we would be knocking on a door. Yes, the potential is certainly there among young people and children. If we have now spoken about doctor-patient communication or communication in the healthcare system, let us then move on to the media and especially social media, which are indeed a gateway for a lot of. I will say cautiously half-knowledge. Formulated mystically. Do you have a suggestion for how it could succeed to critically contextualize medical content in social media without appearing arrogant or preachy? That is, I think, a difficult undertaking in, for example, short TikTok videos or YouTube content. To reach younger people and then. Well, I would be pleased if we would succeed in normal German television to have health programs at a high level. I am a bit on the search for that. Although we see that, especially in the public broadcasting channels, regarding other scientific topics, excellent program formats exist, whether it is history, whether it is biology, cognitive things are looked at, i.e., how the brain works, philosophical things are done, there are excellent formats. I would not know at the moment, if you were to ask me now, is there also a health program that is at that level, whether we would have one. But perhaps I also watch too little television, so that we could first create foundations there. I admit, social media are challenges, because they have yet other laws of their function. But I also believe that one can manage it, if one achieves one thing, even in the short format not only to bubble, the Global-Idea-Kupu-Turnadensyntof, but the exciting thing is the better world. And bringing that into these short formats. One would have to try quite a lot. I can only say in reverse, we have analyses running regarding Instagram on the topic of nutrition and complementary medicine, without already chatting about it before publication, but it looks gruesome. And that is the high risk that we have, because we will now get a generation that will inform itself almost exclusively from such things
informed. So perhaps one must link social media with very successful short-form appearances to a more in-depth offering, where one simply first whets people’s appetite for such things. So, small bites suffice, and the complete menu is made available elsewhere. Sounds like a very good approach. Yes, do you personally have an idea of how structural prerequisites could be created to integrate scientifically grounded complementary offerings into the standard healthcare system? Yes, I believe we must adjust several levers, which are also somewhat typical of the German healthcare system. Demand number one for me would be to abolish the state-examined naturopath. Not the naturopath, otherwise we would have something in the back alleys, but this label “state-examined,” which suggests that something reasonable is happening there or that it is a reasonable and also regulated profession in its practice. There may be fantastic people who really do good things, but there are also many very problematic things. So, that it becomes recognizable for people: Caution, you are in a completely different area here. I also find it catastrophic that many physiotherapists and medical gymnasts are currently becoming naturopaths so that they can reinterpret the inadequately filled prescriptions of doctors. Then they go to strange schools where they learn strange things that are also not evidence-based. So, we must go back and give them another way to also utilize their own very high professional competencies. Second point, we have already achieved one step. Homeopathy is out of the medical continuing education regulations. It will take a long time until it has grown out of our healthcare system, because there are still many younger doctors who cannot simply be deprived of it now. But one must nevertheless keep communicating that this is something different. The next point would be that health insurance companies must not make special contracts for services that are not proven to be evidence-based. So acupuncture, homeopathy, or something like that. That must also go. Also not in the form of private supplementary insurance? In the private sphere, that everyone decides for themselves, I find that okay. I do not forbid anyone from buying and consuming five chocolate cakes every day. But I simply want to make it transparent. I believe everyone may decide for themselves what they want. But not from the public fund. Yes, and I would really, then two different insurances must be involved. You have a very good health insurance in Germany and then you can also buy all sorts of other things on top. That, I find, is completely okay. I do not want to favor anyone. And then it would be very important to me that we abolish the topic of special therapeutic directions, namely that anthroposophic medicine, homeopathy, but unfortunately also phytotherapy would no longer have to provide proof of efficacy. Because we actually had an extremely highly developed phytotherapy in Central Europe, which, like other medications, of course lives from the fact that the manufacturers seriously support research. At the moment, however, they no longer have to prove anything, they become foolish if they conduct a study, because it can only get worse for them, namely that the study does not work. And then you have negative data. You do not want that. So they no longer have a research department and no research budget at all. I am not necessarily in favor of company-funded research. But there must be a step back and say that only what has proof of efficacy comes to market as a medication. Why should that be different for phytotherapy? They can then put the rest on the
Permit dietary supplements, and then comes the final step. Dietary supplements may also be sold in EDK, so not only in drugstores, and in my view, they should no longer be sold in pharmacies. This way, the pharmacist has a set of good products, and patients know that if I want a clean, good preparation, I have a super good contact person here who also works on interaction projects and prepares me for side effects. One thing must be clear to us. There can be no side-effect-free natural medicine, because everything that works can also go wrong sometimes. That is just how it is. May one sometimes wish for something different? Yes, no, I believe we simply have to deal with it. And in the professional interaction between the professional groups, we simply need good mutual support. Definitely. As in all areas of life, communication is everything. It is bad, for example, when in house construction the painter discovers that the wall does not hold when he applies the plaster. Very bad. You have also completed training in hydrotherapy and acupuncture. Exactly, I even have the Curc-Kamie-Party for that. Well, the idea actually comes from the fact that I had a lot to do with oncological patients very early on, and there, of course, the question repeatedly arose, even decades ago, what can I do myself or what have they read up on? By far not to the extent as today. But it happened, and then one is simply interested in it as a young doctor. And the entry was actually phytotherapy, which I consider a very, very solid science. And then one also notices in the state medical chambers' continuing education weeks. Chambers, manual medicine, wh

Ja, also da kommen ganz viele Komponenten zusammen. Da müsste man einiges versuchen. Also Akkupatur, Homo-Partie oder so was. Ich will ja niemanden bevor munden.
