The integration of spiritual needs into clinical communication, particularly in palliative care, helps patients give meaning to their reality and leads to better health outcomes.

In palliative care, communication does not change reality, but helps it to gain meaning. And for this to happen in a meaningful and effective way, it is important to also address the spiritual needs of patients.

The text deals with clinical communication in the context of spirituality, particularly in palliative care. It defines spirituality as the search for meaning, purpose, and connection in life and emphasizes that patients should be understood not only as sick bodies but as whole persons. The assessment of spiritual needs is described as delicate, especially for people without a religious background or with negative experiences through religion. The text refers to studies showing that spiritual care is associated with fewer painkillers, lower anxiety, and better well-being. At the same time, obstacles such as lack of time and the absence of structured frameworks are mentioned. Communication in palliative care does not change reality, but helps it to gain meaning. The text concludes with the appeal to address the spiritual needs of patients with love.

Clinical Communication and Spirituality Science, Ethics, and Religion 05.10.2026 Maria Loupidou Clinical communication is the process of exchanging information, thoughts, and emotions between healthcare professionals and patients or their families. The patient is not merely a sick body; they are a person who has their own way of making sense of life, pain, loss, hope, and the end. Specifically, within the framework of the concept of palliative care, the clinical relationship enables healthcare professionals to take into account the spiritual history of patients. This capacity lies at the heart of substantive care of the patient as a person, strengthens interdisciplinary alignment, and supports patient-centered outcomes. But what is defined as spirituality: Spirituality is the aspect of human nature that concerns the way in which individuals seek and express the meaning and purpose of life, as well as the way in which they experience their connection to the present moment, themselves, others, nature, and the significant or sacred. [1] In simple terms, it is what a patient says in the book by Marie de Hennezel [2]: “I want to live this difficult phase of my life as consciously as possible!” In palliative care, this is achieved when the doctor can listen to the way in which the patient inhabits their world. The exploration of spirituality demonstrates sensitivity towards individuals who do not self-identify as religious or spiritual, individuals with diverse religious backgrounds, or those who have been hurt by religion. In these cases, the question is reconfigured as the team tries to understand who the patient is and what gives meaning to their existence. In any case, the patient does not surrender their entire self as a confession. They offer traces: a phrase, a memory, a fear, a prayer, a silence, a wish. That is why the image accompanying the article is the work by Yannis Kounellis titled Tragedia civile 1975. In the scene set up by the artist, we see a wall covered with gold leaf, an old coat rack with a black coat and a hat, and a lit lamp. Thus, Kounellis does not show the person but their trace. The composition has been interpreted as something reminiscent of a Byzantine iconostasis surface, a space that separates the everyday from the sacred. The small lamp is a small point of light that gives form to presence through absence. Spirituality is found somewhere there: in the way in which a person tries to give meaning to what is happening to them. The benefits of this deeper communication are multiple. Interventions of comprehensive spiritual care have been associated with reduced use of analgesic medications, lower levels of anxiety, and an improved overall sense of relief and well-being. Many patients, especially those who are hospitalized or suffering from incurable diseases, wish to include spirituality in their discussions regarding their care. However, studies show that this rarely happens due to the barriers faced by healthcare professionals, such as the lack of time, a structured framework that explains the discussion – such as within the context of providing palliative care – and a lack of experience in discussing matters of spirituality. Within the framework of palliative care, by helping doctors to explore the sources of meaning, coping, hope, and the spiritual strengths of patients, as well as identifying spiritual issues related to care planning, we help them to document patients’ preferences, clarify the spiritual impacts on health, and better support the design of treatment from their side. Thus, a “spiritual GPS” is activated that helps the patient to better find the paths they will traverse during the difficult period of pain and hardship. In palliative care, communication does not change reality but helps to give it meaning. And for this to be substantive and effective, it is important to address the spiritual needs of patients. I cannot imagine any other way to achieve this than with love.
